As ISH grows, Lynn and I are continuously looking for ways to improve our process for members. My main role is to provide help and support for ISH members. Whether you need help scheduling, accessing member benefits, or just want to connect, I am here for it all.
For any of our new ISH members, I will be sending the weekly member email the last Friday of every month to cover logistical topics, answer frequently asked questions, and provide tips & support. If you have a specific topic/question you would like me to cover in my monthly email, please send me your ideas through this FORM.

CANCELLATION POLICY |

We have revised our cancellation policies. Please take a moment to read the new cancellation policy.
When scheduling new appointments, you will now check a box to confirm you have read and agree to the cancellation policy.

ISH cancelation policy for Lynn Joselyn PA-C, IFMCP, CFSP™ and ISH Nutritionist private medical visits:
In order to serve you best and continue to grow, we require 48-hour notice prior to session cancellation. Cancellations must be within Acuity, using the Cancel Appointment link on your confirmation email, by contacting ISH through Charm message or calling the office phone 970-403-5251.
Please note that appointments canceled with less than 48-hour notice will result in forfeiture of that month's appointment. Clients may reschedule for the following month based on availability. Additionally, failure to attend a scheduled appointment without prior notice will result in forfeiture of that session and include a $60 No-Show Fee for any No-Show visit.
Two No-Shows (or late cancels within 6 hours of appointment time) within one calendar year without just cause, we may exercise the right to refuse continuation of ongoing care.
We appreciate your understanding and cooperation regarding our cancellation policy.

We hear you and understand that having to log in to yet another portal to access your health data, visit summary, message your practitioners, manage supplements and more can feel cumbersome.
There are important reasons we need to use a secure Electronic Health Record such as CHARM, the biggest is to protect YOU and your personal health data AND to keep all of your important information in one place so that we can continue to support you in an efficient way.
In response to feedback, we wanted to share an alternative option for accessing your Charm patient portal that continues to protect your data and keep things in your chart. This may simplify the process for you, give it a try!
The Charm mobile PHR (mPHR) app allows patients to:
- Add and manage health records
- Engage with the Practice or Provider
- Share health passports within and outside the Practice.
- Request medicine and supplement refills.
- Track ailments and the treatment's progress.
- Monitor health vitals and fitness goals.

How to log in to your Charm portal on the mobile app
- Tap on the 'Login' button
- Enter your PHR account 'Username' and 'Password'.
- You will be prompted to enable touch-ID, face-ID, or create a five-digit lock screen PIN to open the application henceforth.
- This is an optional step if you would like to make logging in easier.
